First off, I think this is a bad analogy because I think it’s pretty damn unlikely that Gabe Newell personally wrote the character for and scripted the Medic. Being the head of a video game company is totally different than being an author.
Second, Pretty much what everyone else said. It’s always been pretty obvious that the Medic and the Heavy have a closer relationship than the Medic with anyone else, so it would make sense that they could have a little informal laugh. It’s also obvious that this was in the beginning, timeline wise. Since the uber was pretty much part of the foundation in the game when it first came out, it’s reasonable to assume that there was a gap of time before this video and the actual ‘start’ of the game. Something could have happened to shove it’s self up Medic’s ass unannounced in between that time that would have made him so uptight.
Third, with the doves, it’s called not everything about the character will be handed to you at the beginning. Before the ‘Meet The’ videos, we didn’t know Sniper had trouble with his dad, or lived in a camper van. We didn’t know that the spy was having an affair with the scout’s mother. To reveal little things like that is the purpose of the ‘Meet The’ videos. It gives you insight into the character that you wouldn’t be able to have otherwise.
Fourth, I’ve owned the game since 2008. Unfortunately, the best thing I have to play it on is a laptop that can provide me a glorious five frames per second. Does this stop me from looking into the game? No. Does it stop me from reading through the wiki like a newspaper? No. Does it stop me from knowing all the inside jokes of the game? No. Does it stop me from knowing everything I’d need to know to be an active part of the fandom? No. Does it stop TF2 from being my favorite, longest standing fandom at the moment? Not at all. Astronomers never go into space but they can give you a pretty damn good description of what it’s like.
